Proof of identity:- Elector's photo identity card, Ration card having photograph of the applicant, Passport,Driving licence,AADHAR Card issued by the Unique Identification etc.
proof of address:- Driving licence, Passport, Post office pass book having address of the applicant etc.
proof of date of birth:- Birth certificate issued by the Municipal Authority, Passport etc.
